Music for Psychedelic Therapy
Those of us who've dedicated our lives to this, blending music with the intention of holding expanded states of consciousness, didn't need studies to tell us so. And long before us, healers' songs were already guiding journeys into the depths of the soul. The icaro rising through an ayahuasca ceremony, the drum holding a pulse for hours, the songs at María Sabina's gatherings with the mushrooms in the mountains of Oaxaca: music has accompanied and opened expanded states of consciousness for thousands of years.
What's changed isn't the knowledge itself, but that we can finally measure some of what it does. And when science looks closely, it tends to confirm what many of us already know from experience. In one study on psilocybin for treatment-resistant depression, the quality of the musical experience predicted symptom improvement a week later, while the overall intensity of the drug didn't.
For thirty years, my work was reading dimmed lite rooms and moving them with sound, building narratives and soundscapes that carry people somewhere else. You learn that music doesn't just accompany an experience, it builds it: it decides when the body opens, when it lets go, where the tension sits, when someone finally arrives home. Coming into the world of psychedelic therapy, I recognised that same language immediately. In a session, a good playlist does exactly what a good set does: it guides an emotional arc, only this time turned inward, toward our inner world, our emotions, our memories.

Music isn't background; music is also medicine
In clinical protocols for psychedelic therapy, music doesn't act as ambient sound. The person usually lies down, closes their eyes, turns their attention inward, and, if they are lucky to have a good facilitator, listens to a carefully curated selection throughout the session. Sound sits inside the therapeutic frame, not somewhere outside it.
The evidence describes music as a central therapeutic component precisely because it does several things at once: it supports emotional expression, evokes mental imagery, helps make meaning, and offers a kind of guidance when words fall short. Under the influence of psychedelics, whatever emotion, imagery, or personal meaning the music stirs is intensified.
There's something else, simple but crucial: when music resonates with what a person is living through, it opens a sense of safety and the feeling of being held throughout the process. The right sound doesn't just guide the experience, it accompanies it. It creates the sense that something is holding your hand.
How to design a playlist for a session
A playlist for a psychedelic session isn't just a string of beautiful songs. Classic and current models build it around the temporal arc of the experience, moving through phases of pre-onset, onset, build-up, peak, re-entry, and return, or more condensed versions of that same shape.
Any DJ recognises this structure instantly, because it's the structure of a good set. The opening calls for soft, comforting music that fosters calm and a sense of surrender. The peak is reserved for the most evocative or intense material, and only once a sense of safety has been established. The close returns to calmer sound, supporting the return and the first moments of integration. Recent ketamine trials have used exactly this logic: a serene beginning, a gradual rise in intensity after the first ten or fifteen minutes, and a calm ending.
Methods for evaluating playlists have started to emerge too, combining algorithmic metrics with human judgement. They're useful, but with an important caveat: they still misclassify certain therapeutic qualities, especially in ambient pieces or those built on nature sounds. A track labelled sad or low-energy might be exactly the one that opens a space for tenderness and release. A trained ear still catches things an algorithm misses.
Why the perfect playlist doesn't exist
Here we need to be honest. Despite the models and the phases, the literature doesn't support a single recipe that works for everyone. There's still no robust empirical research comparing standardised playlists against personalised ones, and disagreement persists among experts about how far music should even be tailored to the individual.
Several recent studies describe music as relational and biographical: its effect depends on the listener's history, preferences, culture, and emotional state. This lines up with what we already know about psychedelic therapy more broadly, where context and the therapeutic alliance shape both the acute experience and the clinical outcome. Music, in other words, isn't administered.
It's offered within a relationship, the same way a dose is administered within one. Truly listening to the person in front of you matters just as much as the music you've prepared.

The power of silence
And sometimes what heals isn't sound, but its absence. A recent clinical study with psilocybin built in periods of intentional silence, half an hour at a time. At first, they felt uncomfortable, but they went on to hold mindfulness and dialogue with the therapist, and in one case, they helped someone process difficult memories that the earlier music had stirred up. This should be read with caution, since it comes from just two cases, but it points to a more nuanced, truer idea: music is often valuable, but continuous sound isn't always optimal. Silence is an instrument too. Knowing when to stay silent is part of this craft.
Breathwork, rhythm, and expanded states
Music and sound aren't only relevant when we're talking about substances. Meta-analyses show breathwork is linked to small to moderate reductions in stress, anxiety, and depressive symptoms, though many of the underlying studies carry a moderate risk of bias. High-ventilation breathing accompanied by music can induce altered states that are, in part, comparable to a psychedelic experience. And in practices like group singing done online, the deepest effects, ego dissolution and mystical states, showed up mainly in people with prior experience.
It's a promising direction, but the current state of the evidence needs to be named clearly: it's weaker than what we have for session music, and the mechanisms are still poorly understood. Integration afterward is considered essential across every model, but there isn't yet enough evidence to say which integration framework works best. It's likely that, in time, we'll see a wider range of integration frameworks develop, shaped by the populations they aim to serve. Promising isn't the same as proven, and in this field, that difference matters.

If one thing is clear, it's that music in psychedelic therapy isn't decoration. It's an active, intentional therapeutic tool, built in phases and responsive to the individual. Alongside silence, breathwork, and above all, the integration that follows, it's part of the context that shapes what someone does with what the experience reveals. Science still doesn't justify promising a perfect playlist or a universal formula, and anyone who does is oversimplifying something that is, by its nature, relational.
What research is now documenting is ancient knowledge, the knowledge of ceremonial traditions and of those of us who've spent our lives using sound to accompany others. Transmitted knowledge and measured knowledge, finally shaking hands. That bridge, between lived wisdom and evidence, is the place we work from. And we believe this kind of care, the care of a well-held experience, shouldn't be a privilege.

Why is music used in psychedelic therapy?
Because it doesn't function as background, it's part of the intervention. Music supports emotional expression, evokes imagery, helps make sense of the experience, and offers guidance and reassurance when words fall short. Under a psychedelic, its capacity to stir emotion and meaning is amplified.
Is there an ideal playlist for a psychedelic session?
No. The evidence doesn't support a single formula, and there still aren't studies that robustly compare standardised playlists against personalised ones. What does work is a phased structure, shaped to the moment in the substance's arc, the person's state, and their personal history.
How is a psychedelic playlist structured?
Around the temporal arc of the experience: serene, containing music at the start, a gradual build in intensity, a peak, and a calm close that supports the return. The most evocative material is held back until a sense of safety has already been established.
Does silence also play a role?
